 Case scenario...

Hi, I am Patel Dinesh,3rd year bds student.This is an online elog book to discuss our patients health data after taking her consent.This also reflects my patient centered online learning portfolio. 

CASE SHEET: a 50 year old man with  fever with chills,cough,shortness of breathe.

Chief complaints: Patient came with cough since 20 days ,shortness of breathe since 20days,fever since 7 days 

HISTORY OF PRESENT ILLNESS

Patient was apparently asymptomatic 20days back,then he developed cough with sputum,Mucopurulent , more cough during night,fever since 7 days of intermittent type, chest pain ,shortness of breath since 20days,No Aggregating and Reliving factors 

Burning micturition and Chest pain


PAST HISTORY

Similar complaints 4 months back 

TB 4 months back 

PERSONAL HISTORY

Loss of appetite, mixed diet and addiction of smoking 2 packs beedis per day since 30years stopped from 6 months and consumption of alcohol daily stopped from 6 months 

Family history: nil

Then on 23rd he went to a private hospital where he was investigated and was diagnosed to have jaundice.
